Required Documents :
1. If you participate in another assistance program (SNAP, Medicaid, etc.), sign up is easy! Just bring proof of participation.
2. If not, you will need proof of address, income, and identity to enroll in the ACP Program. Bring one from each category below.
3. All documents must include your first and last name.
